Clinical Care Specialist

The Clinical Care Specialist will be a key member of the Hospitals & Communities Integrated Services (HCIS) or Tertiary Mental Health Services multi-disciplinary care team, ensuring timely and responsive treatment and clinical supports for individuals with serious severe and persistent mental health problems, and/or problematic substance use and/or developmental disability issues.
Key Responsibilities:
• Provide assessment, treatment, case management, community support, and urgent response to clients who may also have serious functional impairment in the domains of physical health and personal, family, financial, occupational, and social life.
• Function as a primary clinician utilizing a variety of specialized treatment modalities/therapies for individuals, groups of clients and/or their families.
• Collaborate with the care team to develop and implement comprehensive care plans that address the unique needs of each client.
