Job Title: NSWOC Nurse

We are seeking a highly skilled and experienced NSWOC Nurse to join our team at CarePartners. As a NSWOC Nurse, you will provide wound and ostomy consultation for patients in the clinic and community, utilizing your expertise in wound and ostomy care to promote self-management and lead the interdisciplinary team to achieve optimal patient outcomes.

Job Summary

The successful candidate will have a strong background in wound and ostomy care, with a focus on providing high-quality patient care and education. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, and provide ongoing support and guidance to patients and their families.

Responsibilities
  • Provide wound and ostomy consultation for patients in the clinic and community
  • Utilize expertise in wound and ostomy care to promote self-management and lead the interdisciplinary team to achieve optimal patient outcomes
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Provide ongoing support and guidance to patients and their families
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
Requirements
  • Registered Nurse (RN) diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending
  • Several years of escalating experience managing wounds and ostomies
  • Strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • Valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • Vulnerable sector check and clear background check
