Mosaic Primary Care Network is committed to providing high-quality patient-centered care. We are seeking an experienced Primary Care Nurse to join our team, who is passionate about delivering exceptional care to patients with primary health care needs, complex care needs, and chronic diseases.
Responsibilities and Accountabilities
- Assess and identify the real/potential risks at the bio-psycho-social-cultural-spiritual levels of persons at risk for or with a chronic condition in order to optimize health and well-being.
- Coordinate and facilitate the management of patients with primary health care needs, complex care needs, and chronic diseases within the PCN.
- Utilize health coaching/case management approach when assessing, planning, implementing, and evaluating care needs.
Education and Licenses
- Bachelor's degree in nursing.
- Licensed with College of Registered Nurses of Alberta (CRNA).
- Current CPR certification.
