Salary

The estimated annual salary for this position is between $106,300 and $133,800, based on a 40-hour workweek and an hourly rate of $51.37 to $64.19.

Job Description

We are seeking a highly skilled Clinical Pharmacist to join our team in Chilliwack, BC. As a key member of our primary care network, you will provide clinical pharmacy services to patients and work closely with healthcare teams to identify and resolve medication-related issues.

Your responsibilities will include:

• Conducting initial and ongoing patient assessments to determine medication needs and potential therapy problems;
• Providing comprehensive medication management and pharmaceutical care to patients;
• Collaborating with healthcare teams to develop patient-specific therapeutic plans;
• Maintaining accurate documentation of patient information and medication records;

Required Skills and Qualifications

To be successful in this role, you will need to possess the following skills and qualifications:

• Current registration as a pharmacist with the College of Pharmacists of British Columbia;
• Two years' recent, relevant patient care experience, ideally in a team-based primary care setting or equivalent combination of education, training, and experience;
• Valid Class 5 driver's license and access to a personal vehicle for business-related purposes;
