You'll work closely with clients, families, and healthcare teams to provide comprehensive care coordination services, develop and implement individualized treatment plans, and facilitate access to specialized services and programs. Key Responsibilities:

- Provide comprehensive care coordination services to clients across various settings
- Develop and implement individualized treatment plans in collaboration with clients, families, and interdisciplinary teams
- Collaborate with community resources and agencies to facilitate access to specialized services and programs
- Participate in quality improvement activities to enhance service delivery and client outcomes
- Engage in ongoing education and professional development to maintain current knowledge and skills in mental health and substance use care

Requirements:

- Bachelor's degree in a relevant health-related discipline (e.g., Social Work, Clinical Counselling)
- 2 years of experience working with clients and families affected by mental illness and substance use disorders
- Eligible for registration or membership with applicable professional associations or regulatory bodies
- Valid Class V BC Driver's License and access to a personal vehicle for business purposes
