About the Role:

As a Community Support Worker with Fraser Health Authority, you'll have the opportunity to make a real difference in the lives of individuals and families affected by mental illness and substance use disorders. You'll work closely with clients, families, and healthcare teams to provide comprehensive care coordination services, develop and implement individualized treatment plans, and facilitate access to specialized services and programs. With opportunities to participate in quality improvement initiatives, contribute to program development, and engage in ongoing education, you'll have a chance to grow professionally and make a meaningful contribution to our team.

Key Responsibilities:

- Provide comprehensive care coordination services to clients across various settings
- Develop and implement individualized treatment plans in collaboration with clients, families, and interdisciplinary teams
- Collaborate with community resources and agencies to facilitate access to specialized services and programs
- Participate in quality improvement activities to enhance service delivery and client outcomes
- Engage in ongoing education and professional development to maintain current knowledge and skills in mental health and substance use care

Requirements:

- Bachelor's degree in a relevant health-related discipline (e.g., Social Work, Clinical Counselling)
- 2 years of experience working with clients and families affected by mental illness and substance use disorders
- Eligible for registration or membership with applicable professional associations or regulatory bodies
- Valid Class V BC Driver's License and access to a personal vehicle for business purposes
