Care Coordinator Assistant is a critical role within Ontario Health atHome, requiring strong communication and interpersonal skills to provide connected, accessible, patient-centred care. Your main responsibilities will include assessing care needs, determining eligibility for services, and developing individual care and service plans.
You will collaborate with patients, caregivers, and healthcare practitioners to link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.
To excel in this position, you will need to have membership, in good standing, with the applicable regulatory body, such as the College of Nurses of Ontario, College of Physiotherapists of Ontario, or College of Occupational Therapists of Ontario. Additionally, you will require 2+ years of recent experience in community health or a related field, knowledge of the healthcare delivery system and community resources, excellent interpersonal and communication skills, and effective time management and prioritization skills.
