Registered Nurse/Registered Psychiatric Nurse

We are seeking a highly skilled Registered Nurse/Registered Psychiatric Nurse to join our Mental Health and Substance Use Services team at Fraser Health Authority. As a key member of our interprofessional care team, you will provide assessment, treatment, consultation, and follow-up services to clients and their caregivers, while collaborating with community agencies to plan and develop services for our client population.
Responsibilities- Provide clinical nursing services on a case management basis to clients and their caregivers, including comprehensive nursing assessments and treatment planning.
- Develop and deliver education programs on mental health and substance use issues to professionals, caregivers, and community groups.
- Carry out treatment plans and interventions, including administering prescribed psychotropic medication and providing support and crisis intervention through home visits and outreach.
- Provide follow-up and monitoring of complex psychiatric/medical situations, including substance use/dependence, and referral services where necessary.
- Consult and collaborate with members of the interprofessional care team to identify and resolve issues and adjust care plans as required.
- Facilitate clients' utilization of appropriate community services by liaising and collaborating with key service providers.
- Document assessments, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Two (2) years recent related experience or an equivalent combination of education, training, and experience.
- Valid Class 5 B.C. Driver's License and access to a personal vehicle with appropriate insurance coverage.
