Job Title: Registered Practical Nurse, Addiction Medication Program

Make a meaningful difference in the lives of individuals struggling with addiction by joining our team at Homewood Health Centre.

We are seeking a skilled Registered Practical Nurse to provide high-quality care to patients recovering from various addictions within a therapeutic milieu environment.

Key Responsibilities:

  • Provide individualized care to patients, prioritizing their needs and well-being.
  • Support the principles of the addiction program, delivered primarily through a group model.
  • Contribute to the development and implementation of patient care plans, ensuring alignment with CNO standards and RPNAO Best Practice Guidelines.
  • Prioritize and function effectively in a fast-paced environment, working collaboratively with interdisciplinary teams.
  • Administer medications and treatments, ensuring patient safety and well-being.
  • Engage with patients, families, and healthcare teams to provide comprehensive care.
  • Participate in clinical discussions, team meetings, and patient/family conferences to ensure seamless care coordination.

Requirements:

  • Licensed as a Registered Practical Nurse (RPN) with the College of Nurses of Ontario, in good standing.
  • 2+ years of experience working with patients with addiction and psychiatric diagnoses, preferred.
  • BCLS for Healthcare Providers.
  • Working knowledge of the Mental Health Act and community mental health resources.
  • Knowledge of CBT/DBT and/or group facilitation, an asset.
  • Ability to manage crisis situations and deal with acutely ill patients.
  • Current knowledge and experience of assessment and treatment of psychiatric disorders, including prevention of suicide and management of aggressive behavior.
  • Understanding of the addicted patient population, treatment methodologies, and medications.
  • Awareness and acknowledgement of the value of using measurement-based care when supporting patients/clients to help guide clinical decision making, experience an asset.
  • Ability to work independently and with a team.
  • Proficient computer skills and experience with working with a number of systems and software applications.
