We are seeking a highly skilled Occupational Therapist to join our team as a Community Liaison. As a key member of our interdisciplinary team, you will play a vital role in assessing patient needs and developing effective treatment plans.

**Key Responsibilities**

* Conduct comprehensive assessments of patients' physical and functional abilities* Develop and implement individualized treatment plans to enhance patients' independence and quality of life* Collaborate with other healthcare professionals to ensure seamless care coordination* Identify patients' need for assistive devices and coordinate delivery and installation*

**Requirements**

* Master's degree in Occupational Therapy (preferred)* Minimum 2 years of experience working as an Occupational Therapist in the field* Strong communication and interpersonal skills*
