Social Worker

Job Summary:
We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Canyon Hospital. As a Social Worker, you will play a vital role in providing quality, client-focused care to our patients and residents.
Key Responsibilities:
- Conduct comprehensive social and psycho-social assessments to identify patient needs and develop effective care plans.
- Collaborate with the interdisciplinary team to develop and implement social work programs and services.
- Provide counseling and support to patients and families, addressing issues related to mental health, relationships, and daily living.
- Act as a resource for patients and families, providing information and referrals to community resources and services.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
- Master's Degree in Social Work from an accredited school of Social Work.
- Three years' recent, clinical experience or an equivalent combination of social work education, training, and experience.
- Current full registration with the BC College of Social Workers.
- Valid Class 5 driver's license and access to a personal vehicle for business-related purposes.
Skills and Abilities:
- Comprehensive knowledge and understanding of counseling skills and clinical practice models.
- Demonstrated ability to conduct social and psycho-social assessments.
- Ability to make effective referrals and work collaboratively with community health and social agency personnel.
- Excellent communication and interpersonal skills.
