Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Fraser Health as a Fracture Liaison Service Nurse. In this role, you will work collaboratively with the interprofessional care team to provide person-centered care to clients with fragility fractures.

Key Responsibilities
  • Assess clients referred to the Fracture Liaison Service and complete intake questionnaires
  • Develop and implement nursing care plans in collaboration with the interprofessional care team
  • Screen eligible clients with low trauma fractures and assess referrals to the Fracture Liaison Service
  • Collaborate with clients and their families/care givers to develop and implement care plans
  • Teach clients and families/care givers about fragility fractures, health promotion, and secondary fracture prevention
  • Document assessments, nursing diagnoses, observations, interventions, and referrals according to practice standards
Requirements
  • Graduation from an approved school of nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
  • Two years of recent, related acute care experience working with a geriatric population experiencing or at risk of experiencing fragility fractures
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes
Preferred Qualifications
  • Ability to communicate effectively with clients, families/care givers, and members of the interprofessional care team
  • Ability to promote person-centered care and demonstrate empathy for clients and families/care givers
  • Ability to integrate and evaluate data from multiple sources to problem-solve effectively
