Job Summary:We are looking for a compassionate and dedicated individual to provide support and care to individuals with developmental disabilities. The successful candidate will be responsible for implementing life skills programs, providing tailored support, and providing appropriate personal and physical care.
Responsibilities:- Participate in life planning sessions to develop and implement individualized support plans
- Implement life skills programs to enhance the capacity of individuals to function in all aspects of community living
- Provide tailored support to meet the unique needs of each individual
- Provide appropriate personal and physical care, including medication administration and crisis response
- Liaise with friends and family to address needs and share information on behalf of individuals supported
- Maintain documentation and ensure an appropriate standard of living for individuals supported
- Minimum of 6 months of work experience in a related field
- Knowledge of health-care procedures and mechanical equipment, including wheelchairs, lifts, and G-tubes
- Valid driver's license is an asset
- DSW Certificate, Criminal Police Background Check, CPR First Aid, CPI, and up-to-date immunizations
