Physiotherapist - Community Care Specialist

About the Role:
CarePartners is seeking a skilled Physiotherapist to join our dynamic Rehabilitation Team in a Part-Time capacity. As a Physiotherapist - Community Care Specialist, you will play a critical role in providing high-quality patient care in the community and in patients' homes.
Key Responsibilities:
- Conduct professional assessments of patients in the community and in their homes to develop effective care plans.
- Work collaboratively with patients, families, and healthcare teams to establish measurable goals and outcomes.
- Provide one-on-one therapeutic support to clients, promoting independence and empowering them to manage their health in their own homes.
- Participate in interdisciplinary care teams to ensure comprehensive and coordinated care.
- Contribute to the development and implementation of community-based rehabilitation programs.
Requirements:
- Master's Degree in Physiotherapy from a recognized university.
- Registration with the College of Physiotherapists of Ontario.
- Proven computer skills, including proficiency in electronic health records and scheduling systems.
- Strong assessment and treatment skills, with the ability to work independently and as part of a team.
- Excellent verbal and written communication skills, with the ability to work effectively with diverse populations.
- A reliable vehicle for regular travel to patients' homes and community locations.
