About the Role

We are seeking a highly skilled Nurse Practitioner to join our Palliative Care Consultation Team. As a key member of our team, you will play a vital role in providing high-quality, patient-centered care to individuals with complex palliative needs.

Key Responsibilities
  • Assess and manage patients with complex palliative needs, including those with chronic pain and symptom management issues.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.

  • Participate in interprofessional teams to coordinate care and ensure seamless transitions between healthcare settings.
  • Contribute to quality improvement initiatives and best practice development within the team.
Requirements
  • Current registration with the College of Nurses of Ontario.
  • Specialized education and training in palliative care, including pain and symptom management.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong assessment and problem-solving skills, with the ability to think critically and make sound judgments.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
