About Our Unit:
The Urban Health Acute Care Unit is a 24-bed unit that specializes in caring for patients with infectious diseases, addiction, HIV/AIDS, and chronic/acute illnesses associated with marginalization. Our team is dedicated to providing patient-centered, trauma-informed care that addresses the unique needs of our patients. We prioritize harm reduction approaches and work collaboratively to ensure seamless transitions between hospital care and community support services.
Your Role:
As an LPN, you will be responsible for assessing patients' physical, social, spiritual, and psychological needs, developing individualized care plans, and implementing evidence-based practices. You will collaborate with the interdisciplinary team to provide holistic care, address learning needs, and advocate for patients' rights. Your duties may include administering medications, monitoring signs/symptoms, preparing patients for procedures, and participating in care planning meetings. You will also contribute to staff orientation and education, ensuring continuity of care and promoting best practices within our team.
Key Skills & Qualifications:
To succeed in this role, you must possess excellent communication and interpersonal skills, as well as a commitment to providing compassionate, patient-centered care. You should have experience working with diverse patient populations, particularly those with complex health needs. A strong foundation in medical-surgical nursing, infection control, and medication management is essential. Additionally, you should demonstrate proficiency in using electronic medical records, reporting incidents, and maintaining accurate documentation.
