Job Summary
The Social Worker works as a member of the interdisciplinary team to provide social work support and discharge planning services. The Social Worker provides in-depth psychosocial assessments and appropriate interventions for patients and their caregivers, including resource education, emotional support, counseling, case management, and advocacy, to maximize patient quality of life within their community. They will coordinate comprehensive discharge plans with the interprofessional health care team for primarily complex patients and their families.
Primary Responsibilities
1. Coordinate complex discharge plans and provide linkages to community resources to support patients, families, and caregivers transitioning through the continuum of care.
2. Conduct comprehensive and coordinated psychosocial assessments to identify patients' needs and develop effective care plans.
3. Participate in daily bullet rounds, interdisciplinary team rounds, and corporate joint discharge rounds to facilitate patient care planning and coordination.
4. Assist patients, families, and caregivers to understand the impact of their illness or disability on their lives.
5. Provide short-term crisis and grief counseling to patients, families, and caregivers as needed.
6. Educate patients, families, and caregivers about their plan of care, including discharge, and provide information and resources to support their recovery.
7. Maintain accurate documentation of social work services according to the Ontario College of Social Workers and Social Service Workers (OCSWSSW) standards.
8. Take primary responsibility for complex discharge planning across the cardiac department, ensuring seamless transitions and optimal outcomes.
9. Utilize hospital systems, policies, and processes to facilitate sustainable and durable discharges, minimizing readmissions and improving patient satisfaction.
10. Collaborate closely with Patient Flow Navigators, Charge Nurses, Discharge Planners, Clinical Coordinators, and Managers to ensure effective communication and coordination throughout the discharge process.
11. Identify potential barriers to discharge and work with the interdisciplinary team to address these challenges, ensuring timely and safe discharges.
12. Connect patients with community resources and services as required to support their ongoing care and well-being after discharge.
Qualifications
1. Master's Degree in Social Work from an accredited university is required.
2. Membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), with no terms, conditions, or limitations, is mandatory.
3. A minimum of three years of relevant experience in social work or discharge planning within an acute care setting is preferred.
4. Proven ability to conduct comprehensive assessments and develop treatment plans, resolving conflicts and problems for both inpatients and outpatients.
5. Working knowledge of relevant legislative acts, including the Mental Health Act, Long Term Care Homes Act, Retirement Homes Act, Child Youth and Family Services Act, Health Care Consent Act, and Substitute Decisions Act.
6. Demonstrated expertise in psychosocial assessment, conflict resolution, and creative problem-solving to optimize patient outcomes.
7. Ability to work collaboratively with other team members, developing effective partnerships with community agencies to support patient care.
8. Strong communication and interpersonal skills, with the capacity to work effectively with patients, families, interprofessional team members, and other stakeholders.
9. Excellent organizational and coordination skills, with the ability to prioritize competing workload demands and manage multiple projects simultaneously.
10. Flexibility and adaptability to work in a fast-paced environment, responding to changing priorities and deadlines as needed.
11. Commitment to patient and staff safety policies and procedures, fostering a safe and inclusive environment for all.
12. Regular and reliable attendance is an essential requirement of this position.
