Key Responsibilities- Conduct in-home assessments and create personalized care plans for clients
- Consult with physicians and other care providers to ensure comprehensive care
- Supervise and monitor the care delivered by our team of caregivers
- Participate in quality assurance and risk management programs to ensure the highest standards of care
- Collaborate with clients, families, and staff to develop and implement effective care plans
- License practical nurse
- Current certificate of competence and in good standing
- Strong clinical skills and ability to work with minimal supervision
