Job Summary
We are seeking a skilled RN Case Manager to join our team of dedicated healthcare professionals. The successful candidate will be responsible for providing comprehensive case management services to patients and their families, ensuring seamless coordination of care and promoting optimal quality of life.
Responsibilities
- Assess patient needs and develop individualized care plans, working collaboratively with interdisciplinary teams to ensure effective implementation and monitoring.
- Foster open communication with patients, families, and healthcare providers, promoting transparency and trust throughout the care process.
- Provide education and support to patients and their families, empowering them to make informed decisions about their care and promoting self-advocacy.
- Collaborate with colleagues to develop and implement best practices, enhancing the quality of care delivered and advancing the mission of Gentiva Hospice.
- Contribute to quality improvement initiatives, using data-driven insights to drive change and promote continuous learning within the organization.
Requirements
- RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
- Current driver's license and automobile insurance.
- Current CPR certification.
