Job Summary

Vancouver Coastal Health is seeking a Licensed Practical Nurse to join the Home Health team at Community Health in North Vancouver.

About the Role

As a Licensed Practical Nurse with VCH, you will provide care to clients and families based on predictable outcomes and work collaboratively as a member of an interprofessional team.

Responsibilities
  • Practice collaboratively as a member of an interprofessional team
  • Provide care to clients and families based on predictable outcomes
  • Work closely with the RN in direct care activities
  • Support clients and families to achieve and maintain optimal health
About Home Health Programs

Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.

Qualifications
  • Graduation from a recognized program for Practical Nurses
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
  • Three (3) years recent experience working as a Licensed Practical Nurse providing care in the community or acute (medical/surgical/palliative, simple wound care) care setting
