Job Summary:

The Physiotherapist is a key member of our multidisciplinary team at Home and Community Care Support Services, responsible for providing high-quality physiotherapy services to patients in their homes and communities.

Key Responsibilities:

  • Assess and develop individualized physiotherapy plans to address patients' physical disabilities and impairments.
  • Provide evidence-based physiotherapy interventions to improve patients' functional independence and safety in their environments.
  • Collaborate with healthcare professionals, case managers, and community organizations to ensure comprehensive care and support.
  • Conduct thorough assessments and evaluations to identify patients' needs and develop effective treatment plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Stay up-to-date with best practices and research in physiotherapy to provide the highest quality care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements:

  • Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario.
  • Strong knowledge of physiotherapy services and community resources.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and access to a vehicle.
  • Advanced oral and written proficiency in English is essential, French would be considered an asset.
