Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team as a Hospice Care Specialist. In this role, you will provide safe and respectful nursing care to patients with complex or unpredictable outcomes, working collaboratively with the RN/RPN and other members of the interdisciplinary care team. Your expertise will be essential in supporting patients and their families through the end-of-life journey, promoting dignity, comfort, and quality of life.

Responsibilities
  • Conduct thorough assessments, monitor patient status, and communicate with the RN/RPN as needed.
  • Develop and implement individualized care plans, prioritizing patient needs and preferences.
  • Administer medications, including narcotics, according to established procedures and monitor their effects.
  • Collaborate with the interdisciplinary team to provide comprehensive care, including education and support for patients and their families.
  • Participate in staff meetings, special projects, and hospital committees to contribute to the development of patient care and nursing practice.
Requirements
  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
  • Current basic cardiac life support certification.
