Job Summary

The Rehabilitation Assistant will provide treatment to patients who have been discharged from hospital with various illnesses, impairments, and injuries. This program focuses on high-intensity rehabilitation, including walking programs, transfer training, strengthening, and endurance training.

Duties and Responsibilities:

  • Implements rehabilitation programs with clients as directed by Physiotherapists.
  • Promotes maximum independence of clients in the home, community, and at work to support re-integration into the community.
  • Implements exercise programs or group exercise classes as directed.
  • Facilitates and supervises activities of daily living as directed.
  • Facilitates and implements social re-integration as directed.
  • Facilitates and implements educational and vocational re-integration as directed.
  • Facilitates and implements cognitive therapy programs as directed.
  • Promotes and implements leisure activities as directed.
  • Attends case conferences and team meetings as required.
  • Drives clients as directed (with valid insurance coverage).
  • Understands and implements infection prevention practices.
  • Participates in ongoing internal and/or external continuing education activities.
  • Adheres to Bayshore HealthCare policies and procedures.
  • Participates in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Participates in proactive Health & Safety activities while performing all duties.
  • Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore HealthCare personnel.
