We are seeking an experienced Social Worker to join our team at the Intensive Care Unit (ICU) at the Kelowna General Hospital (KGH).
As a Social Worker, you will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families. You will work collaboratively with other healthcare providers and the multidisciplinary team to provide direct psychosocial care, counseling, system navigation, and advocacy, education, and linkage to relevant specialized complex care resources while facilitating client self-management.
Key Responsibilities:
Provide comprehensive assessment and care planning for clients and their families
Develop and implement treatment plans to meet the needs of clients
Collaborate with healthcare providers and the multidisciplinary team to provide direct psychosocial care
Provide counseling, system navigation, and advocacy to support clients and their families
Educate clients and their families on relevant health and social issues
Link clients to relevant specialized complex care resources
Facilitate client self-management and empowerment
Requirements:
Bachelor of Social Work degree or equivalent
Experience working in a healthcare setting
Strong assessment and care planning skills
Excellent communication and interpersonal skills
Ability to work collaboratively with healthcare providers and the multidisciplinary team
Ability to provide direct psychosocial care and counseling
Ability to navigate complex healthcare systems
Ability to educate clients and their families on relevant health and social issues
Ability to link clients to relevant specialized complex care resources
Ability to facilitate client self-management and empowerment
