LPN - Skilled Nursing Position

The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for coordinating care delivery and providing individualized resident-centered care. This role requires a strong commitment to delivering high-quality care and a passion for working with seniors.
Responsibilities:- Assessment and Care Planning: Conduct thorough assessments of residents' needs and develop individualized care plans to ensure their physical, emotional, and social needs are met.
- Medication Administration: Administer prescribed medications and treatments within the scope of practice, ensuring accurate and timely delivery.
- Wound Care: Assess and treat wounds on the unit, providing education and support to care staff to prevent skin breakdown.
- Communication and Collaboration: Liaise with physicians, community agency representatives, families, and other disciplines to ensure seamless care delivery.
- Documentation and Reporting: Accurately document changes in residents' conditions and report to family and physician as needed.
- Team Leadership: Provide coaching and direction to care staff, promoting a culture of excellence and resident-centered care.
- Professional Development: Participate in ongoing education and training to maintain current knowledge and skills.
- Education: Graduate of an approved program for Practical Nursing and current licensure with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
- Certifications: Valid Occupational First Aid Level 1 certification and successful annual Resident Assessment Instrument (RAI) competency testing.
- Experience: Previous experience in a long-term care facility is an asset.
- Personal Qualities: Demonstrated collaborative and team working skills, ability to read, write, and speak English proficiently, and computer skills including basic navigation of Word, Excel, and Outlook.
