Job Summary

Key Responsibilities
  • Conduct thorough psycho-social assessments of clients and their families to identify their emotional, social, and physical needs.
  • Provide clinical counseling services to clients and their families to address issues related to chronic illness, disability, relocation, bereavement, and coping strategies.
  • Collaborate with interdisciplinary teams to develop and implement client care plans that address their unique needs and circumstances.
  • Facilitate interdisciplinary team conferences and meetings to ensure seamless communication and coordination of care.
  • Provide education and consultation to clients, families, and healthcare professionals on issues related to high-risk factors, adaptation to illness and disability, loss, grief, and bereavement, transition stress, protection of vulnerable adults, and critical incident stress management.
  • Maintain ongoing knowledge of community services and resources available to support clients and their families.
  • Contribute to research activities and participate in program planning and development to advance social work practice and improve client outcomes.
Requirements
  • Bachelor's Degree in Social Work from an accredited School of Social Work.
  • One year's recent, related experience or an equivalent combination of education, training, and experience.
  • Current full registration with the BC College of Social Workers.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to work collaboratively as part of an interdisciplinary team and understand the frameworks of other disciplines.
  • Strong knowledge of community resources and services, with the ability to navigate complex systems and advocate for clients' needs.
