About the Role

We are seeking a highly skilled and compassionate Social Worker to join our team at Homewood Health, a leading provider of mental health and addiction services in Canada.

Key Responsibilities
  • Provide client-centered treatment and recovery-oriented support to clients with complex mental health needs.
  • Conduct comprehensive client assessments, including in vivo discipline-specific assessments and multidisciplinary assessment instruments.
  • Develop individualized client treatment/recovery plans in collaboration with clients and other stakeholders.
  • Assume responsibility for the treatment, recovery process, and support of assigned clients within the context of primary worker and the larger team.
  • Provide direct assistance with symptom management, including delivery of medication and 24-hour crisis support.
  • Facilitate an optimally supportive environment, including assistance with meeting basic needs and promoting self-advocacy and client self-determination.
Requirements
  • Master's Degree in Social Work.
  • Registration with the College of Social Workers.
  • Training and education in CBT, CPT, and DBT, with hands-on work experience preferred.
  • Experience working with complex/comorbid mental health conditions and providing evidence-based approaches to treat them.
  • Exceptional communication and interpersonal skills.
  • Effective computer skills.
  • Strong report writing skills with a high level of attention to detail.
