Job Summary:
We are seeking an experienced Nurse or Licensed Practical Nurse (LPN) to join our team as a Healthcare Support Specialist. In this role, you will be responsible for providing exceptional care and support to our clients, working collaboratively with healthcare professionals to ensure the highest level of service delivery.
Responsibilities:
- Provide nursing care and support to clients in accordance with their individualized program plans
- Assess and manage client needs, collaborating with healthcare professionals to develop and implement effective care plans
- Communicate effectively with clients, families, and colleagues to ensure seamless care coordination
- Participate in on-going training and professional development to enhance skills and knowledge
- Contribute to a positive and inclusive work environment that fosters teamwork and collaboration
Requirements:
- Diploma or degree in Nursing or related field
- Current registration as a Nurse or LPN with a valid Class 5 driver's license
- Experience working with individuals with diverse abilities and complex needs
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
