Job Summary
The Registered Dietitian - Facility Based is a key member of the Prairie Mountain Health team, responsible for providing high-quality nutritional care and counseling to patients, residents, and clients. This role requires collaboration with healthcare teams, including patients, residents, clients, and next of kin, to develop and implement effective nutrition care plans.
Key Responsibilities
- Conduct comprehensive nutrition assessments to identify individual needs and develop personalized care plans.
- Provide nutrition education and counseling to patients, residents, and clients to promote healthy eating habits and disease management.
- Collaborate with healthcare teams to develop and implement nutrition-related policies and procedures.
- Participate in multidisciplinary team meetings to discuss patient care and develop action plans.
- Contribute to the development of educational materials and programs related to nutrition care.
- Prepare statistical and status reports for the Area Manager to track progress and identify areas for improvement.
Requirements and Qualifications
This role requires a strong background in nutrition and dietetics, as well as excellent communication and collaboration skills. The ideal candidate will be a registered dietitian with experience working in a healthcare setting and a passion for delivering high-quality patient care.
