VHA Home HealthCare is seeking a highly skilled Occupational Therapist to join our team in the St. Thomas and Elgin County Area. As an Occupational Therapist at VHA, you will play a vital role in assessing and providing consultation, recommendations, and interventions to a diverse group of pediatric clients in their homes and/or schools.
Key Responsibilities- Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the relevant regulatory college.
- Implement client interventions based on assessment information, client input, and in conjunction with the client's integrated care plan.
- Ensure that services provided are best practice, evidence-based, and evaluated through the routine use of valid and reliable outcome measures.
- Ensure coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
- Document all assessment findings, treatment plans, progress, and discharge summaries of interventions as required by the profession, referral source, and the organization.
- Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
- Degree in Occupational Therapy from a recognized school of Occupational Therapy.
- Member in good standing with the College of Occupational Therapists of Ontario.
- Valid Driver's License with access to a vehicle.
- Compassionate and empathetic.
- Self-driven with ability to work independently.
- Skilled in caseload management.
- Skilled at electronic documentation.
- Able to provide virtual care.
- A registered ADP authorizer (asset).
