Care Coordinator

We are seeking a skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing patient care needs, determining eligibility for services, and providing access to community resources.
Key Responsibilities- Assess patient care needs and develop individual care plans
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families
- Balance patient needs and choices with available resources
- Membership in good standing with the applicable regulatory body
- University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work
- 2+ years of recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
