Job Description

We are seeking an Advanced Healthcare Provider - Primary Care Specialist to join our team at Shellbridge Family Health Clinic. As a key member of our interdisciplinary healthcare team, you will be responsible for delivering high-quality patient care, collaborating with colleagues to develop comprehensive care plans, and contributing to the growth and development of our clinic.

Key Responsibilities:

  • Comprehensive Assessment: Conduct thorough assessments of patients' physical and mental health needs, including diagnosing conditions and developing treatment plans.
  • Collaborative Care: Work closely with other healthcare professionals to provide holistic care, coordinate services, and ensure seamless transitions between care settings.
  • Patient Education: Educate patients and their families on healthy lifestyles, disease management, and self-care strategies to empower them to take control of their health.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes, improve care processes, and promote best practices within the organization.
  • Professional Development: Stay current with the latest advancements in primary care, participate in ongoing education and training, and contribute to the professional growth and development of colleagues.

Required Skills and Qualifications

To succeed as an Advanced Healthcare Provider - Primary Care Specialist, you will require:

  • Current Registration: Registration with the British Columbia College of Nurses and Midwives (BCCNM) as a Nurse Practitioner.
  • Clinical Experience: A minimum of four years of recent, relevant clinical nursing experience, preferably in primary care or a related field.
  • Education: Completion of a Master's degree in Nursing or a related field, and successful completion of the Objective Structured Clinical Examination (OSCE).
  • Leadership Skills: Strong leadership and communication skills, with the ability to collaborate with colleagues, patients, and families to achieve shared goals.
