We are seeking a skilled and compassionate Community Health Specialist to join our team at CarePartners. As a Community Health Specialist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide exceptional nursing services to patients in a variety of settings, including in-home, clinics, and retirement homes
  • Communicate and liaise with patients, family members, and other members of the interdisciplinary care team
  • Perform other clinical duties as required

Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma

  • Exceptional interpersonal skills
  • A valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
