About the Role:

We are seeking a dedicated and compassionate Registered Nurse to join our team in providing high-quality care to patients in their homes. As a Registered Nurse in our community nursing role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

  • Conduct thorough assessments of patients' needs and develop personalized care plans
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient progress and maintain accurate records

Requirements:

  • Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse
  • Valid driver's license and access to a reliable vehicle
  • Experience in community care or a related field is an asset
  • Excellent communication and interpersonal skills
