At Lifemark Health Group, we are committed to providing comprehensive rehabilitation services to Canadian Veterans. As a Primary Care Physician, you will play a critical role in our team's efforts to support Veterans in their transition to life after service.

Key Responsibilities:

  • Conduct thorough assessments of presenting eligible health problems
  • Perform medication reviews and identify drug therapy problems
  • Clarify relevant medical issues, including comorbid conditions
  • Clarify diagnosis and prognosis related to eligible health problems
  • Identify conditions that may hinder progress in rehabilitation
  • Collaborate with Veterans, support persons, and family members to promote understanding of medical conditions and recommendations
  • Communicate with primary healthcare providers to collaborate on rehabilitation care
  • Engage in targeted follow-up care through a consultative model
  • Collaborate with PCVRS Medical Director(s) and engage in continuous quality improvement, learning, and skills development

Requirements:

  • Doctor of Medicine and valid license to practice in respective field in province/territory as applicable
  • Member in good standing with provincial regulatory body
  • Experience in managing complex medical conditions
  • Bilingual (French/English) is an asset
  • Experience working with a variety of health professionals and disciplines in an interdisciplinary team environment is an asset
