Nurse Care Specialist

Job Summary:
Closing the Gap Healthcare Group is seeking a skilled Registered Nurse or Registered Practical Nurse to join our team. As a member of our community care team, you will provide high-quality care to clients in their homes and communities.
Key Responsibilities:
- Provide comprehensive care to clients, including wound care management and pediatric care.
- Assess client needs and develop individualized care plans.
- Collaborate with healthcare teams to ensure seamless care delivery.
- Communicate effectively with clients, families, and healthcare providers.
- Maintain accurate records and reports.
Requirements:
- Current certification as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the CNO.
- Minimum of two years' community experience.
- Experience with pediatric clients and wound care management is an asset.
- Standard First Aid & CPR/AED (Level C) certification.
- Mask Fit test certification.
