We are seeking a skilled Registered Nurse (RN) to join our Home Living team, providing professional support services to clients of all ages and stages of life. As a self-directed team member, you will work collaboratively with the healthcare team to assess client needs, deliver appropriate therapeutic nursing services, coordinate resources, establish goals for client delivery, and evaluate outcomes.

This role also involves case management, direct client care that meets established clinical standards, and evidence-based practice. You will act as a consultant on the team, providing clinical expertise and direction to other team members regarding client cases. Your contributions will help develop a comprehensive care plan, utilizing internal and external resources as needed.

In addition to your clinical responsibilities, you will participate in staff development and mentor student clinical experiences. This position is based out of the Fort Saskatchewan Home Care office but may require travel to other HC network offices as needed.

Job Description:

As a Registered Nurse, you will provide a wide range of nursing services to patients, families, communities, and populations while prioritizing their safety and well-being. You will utilize nursing processes, including critical thinking, problem-solving, and decision-making, to teach, counsel, and advocate on behalf of patients and their families.

You will play a key leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance. Your primary focus will be providing safe, quality patient-centered care that reflects the shared vision and values of AHS.

Required Skills and Qualifications:

  • Completion of an accredited nursing education program
  • Active or eligible registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
  • Relevant experience in community sector nursing, preferably with palliative care, continuing care, and geriatric experience
  • Proficiency in Meditech, Connect Care, and RAI Home Care assessment tool
  • Two years of current Case Management Experience in Continuing Care

Estimated Salary: $

$94,499 - $120,379 per year, depending on qualifications and experience. PLEASE NOTE: Provisional Permit Holders shall be paid upon being entered into the register of Registered Nurses.
