Pacific Coast Community Resources is seeking a highly motivated and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will work closely with individuals with disabilities to provide care and support in their community.
Key Responsibilities- Empowerment and Advocacy: Ensure the rights and choices of individuals are respected and advocate as needed to promote their independence and self-determination.
- Community Engagement: Foster positive relationships with individuals, family members, professionals, and community members to promote a sense of belonging and connection.
- Personalized Support: Develop and implement personalized service plans to meet the unique needs and goals of individuals.
- Health and Safety: Ensure the health and safety of individuals by providing assistance with daily living activities, medication administration, and maintaining a clean and safe living environment.
- Communication and Documentation: Record daily activity and personal health information (PHI) of individuals on our case management database.
- Education: Grade 12 education and a Community Support Worker Diploma or equivalent.
- Experience: Minimum 2 years of experience in community social services or healthcare.
- Certifications: Current Emergency First Aid/CPR and Food Safe certificates, previous behaviour management course/training, and a restricted class 4 driver's license.
- Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and moderate computer literacy.
- Competitive Salary: $25.76 per hour.
