Job Summary

This permanent part-time position offers an exciting opportunity to join our Patient Services team as a Patient Care Coordinator Assistant. You will play a vital role in supporting our Care Coordinators, ensuring seamless patient care delivery.

Duties and Responsibilities

  • Support Care Coordinators in their daily activities, ensuring timely completion of tasks and efficient service delivery.
  • Maintain accurate and up-to-date patient records, adhering to confidentiality guidelines.
  • Enter, update, and manage patient data in our electronic database.
  • Provide excellent customer service to patients, families, and service providers via phone and email.
  • Collaborate with other departments as needed to ensure smooth workflow and efficient service delivery.

Requirements

  • A college diploma in health or social services, business/office administration, or a related field.
  • At least 2 years of experience in an office environment, preferably in healthcare administration.
  • Exceptional keyboarding/data-entry skills, proficiency in MS Word, Excel, and other applications.
  • Excellent organizational and time management skills, ability to work independently with minimal supervision.
  • Familiarity with medical terminology and office procedures, knowledge of services provided by Local Health Integration Networks.
