**About the Role:**

We are seeking a highly skilled and compassionate Licensed Practical Nurse to join our team at Optima Living. As a key member of our care team, you will be responsible for providing high-quality nursing care to residents in our Independent Living, Assisted Living, Supportive Living, and Memory Care communities.

Key Responsibilities:

  • Develop and implement individualized care plans under the direction of the Director of Care or designate, focusing on the physical, psychological, and social needs of residents.
  • DIRECT and assist care aides in adhering to established procedures, ensuring exceptional resident care standards.
  • Administer, monitor, and record medication administration, educating residents as needed.
  • Collaborate with healthcare professionals to create and manage nursing care plans, participating in the evaluation of current and future medical needs.
  • Evaluate the effectiveness of nursing care treatments and inform the Health & Wellness Manager or designate and/or the resident's physician of significant changes in resident condition.
  • Provide personal care assistance, nursing procedures, mentorship, and coaching to Health Care Aides.
  • Work closely with residents to meet care requirements and communicate any concerns to the doctor and manager as needed.
  • Demonstrate ongoing competence by utilizing knowledge, skill, judgment, and evidence-based practice in caring for aging adults.
  • Ensure adherence to provincial continuing care health service standards, legislation, codes, regulations, and employment and safety statutes.
  • Demonstrate awareness of accident and injury prevention through safe work practices and procedures.

Requirements and Qualifications:

  • Nursing diploma completion.
  • Registration with the provincial nursing regulatory body and good standing.
  • Experience in a residential care setting is preferred.
  • Demonstrated infection control procedures.
  • Exceptional interpersonal skills with peers, residents, visitors, and operational partners.
  • Superior organizational skills with the ability to handle multiple resident requests.
  • Superior oral and written communication skills.
  • Demonstrate ability to prioritize and problem-solve.
  • Flexibility to work various shifts.
  • Ability to stand for extended periods, bend, squat, kneel, and lift 25 pounds.
  • Display compassion, education, proficiency, and experience while assisting residents.
