We are looking for a highly motivated and detail-oriented Regional Geriatric Program Consultant to join our team at St. Joseph's Health Care London. As a key member of our team, you will be responsible for utilizing quality improvement and co-design approaches to support the development and implementation of evidence-based strategies that improve health system outcomes for older adults and their caregivers.
The successful candidate will have a strong background in quality improvement, knowledge translation, and patient-centered care. You will work closely with hospital staff, physicians, older adults, caregivers, and community stakeholders to implement best practice strategies, ensuring measurable and sustainable results.
Key Responsibilities:
- Support and manage process improvements/lean projects
- Implement co-design approaches and work with stakeholders to develop and implement evidence-based strategies
- Assess and evaluate program outcomes and identify opportunities for quality improvement
- Collaborate with cross-sectoral partner organizations and Ontario Health Teams to translate geriatric best practices and resources into actionable materials
Preferred Qualifications:
- Proficiency in French would be an asset
