Job Title: Unit Coordinator

North York General Hospital is seeking a highly skilled and experienced Unit Coordinator to join our Reactivation Care Centre team. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and ensuring seamless transitions throughout the care journey.

Key Responsibilities:
  • Provide day-to-day assistance and leadership to the inter-professional team, ensuring effective communication and collaboration.
  • Collaborate with patients, caregivers, and the inter-professional team to support discharge planning and timely transitions in care.
  • Participate in unit-based and corporate committees to drive quality improvement and best practices.
  • Conduct research, create presentations, and posters relevant to the unit and program, promoting evidence-based practice and innovation.
  • Develop and implement systems and processes to optimize unit functioning and patient flow.
  • Assist in the implementation and evaluation of targets, goals, and objectives, ensuring alignment with hospital and external priorities.
  • Support staff in utilizing critical thinking skills to resolve clinical and administrative issues, promoting a culture of excellence and patient safety.
  • Provide direction and guidance to all members of the healthcare team, patients, and their families, promoting a patient-centered approach to care.
  • Engage in evidence-based practice, identifying opportunities to improve the quality of patient care and implementing changes as appropriate.
Requirements:
  • Possess a valid Certificate of Competence from the College of Nurses of Ontario.
  • Baccalaureate in Nursing required, with a Master's degree completed or in progress.
  • 3-5 years recent experience in medical/surgical and/or nursing experience with older adult, dementia, and cognitively impaired patient populations preferred.
  • 3-5 years recent experience in restorative care and rehabilitative care for the elderly and palliative care services preferred.
  • 3-5 years leadership experience (Charge Nurse) with a good understanding of discharge planning and community supports.
  • Quality improvement and teaching experience preferred.
