About the Role:
Halton Healthcare is seeking a highly skilled Nurse Practitioner to join our team in providing exceptional care to individuals with mental health and substance use disorders. As a key member of our inter-professional team, you will utilize your clinical expertise and in-depth knowledge to plan and evaluate patient care.
Key Responsibilities:
- Provide specialized assessment, coordination, and treatment for individuals living with mental health and substance use disorders.
- Deliver counseling, education, and support services to individuals, families, and the community.
- Work collaboratively with an inter-professional team within the Mental Health Program and establish partnerships with related community organizations.
- Develop and implement individualized treatment plans that address the unique needs of clients.
- Provide opioid addiction treatment with a harm reduction approach to care.
Requirements:
- Master's Degree in Nursing or recognized equivalent.
- Current registration in the Extended Class with the College of Nurses of Ontario.
- Nurse Practitioner's Certificate from a recognized program.
- Minimum three years' experience working in the field of substance use disorders/addiction.
- Minimum five years of experience working in the field of Mental Health, including both inpatient and community-based services.
