Our Complex Care Support Team is currently seeking a Geriatric Psychiatry Specialist with expertise in dementia care to join our multidisciplinary team. In this role, you'll provide comprehensive care to adult patients across various care settings while closely collaborating with a team of healthcare professionals. You'll utilize best practices to optimize patient care and support individuals, families, and primary care providers in the North Zone. As a leader and resource in your field, you'll mentor fellow team members, lead research initiatives, maintain effective relationships with stakeholders, and engage with clients and families as partners in care. This position involves both face-to-face patient interactions and virtual consultations with clients and the interdisciplinary team.

Description:

As a Geriatric Psychiatry Specialist, you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems.

Required Qualifications:

Successful completion of an approved Nurse Practitioner program and a minimum of master's degree. Registration with CRNA on the Nurse Practitioner Registrar including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Experience in at least 2 care sectors, including Seniors Health. CNA Certification in Geriatrics, or willingness to obtain. Case Management Education. Experience in program planning, education of adults, interdisciplinary practice, and research activities. Excellent interpersonal & communication skills. Ability to work independently & to be flexible to adapt to changing clinical situations. Works collaboratively with all members of the health care team, including patients and their families.
