About the Role

The Social Worker St. Mary's Health @Home (SW-SM H@H) program is designed to provide psychosocial support and care to individuals who have been discharged from hospital and require ongoing clinical care in their own homes.

This role will work closely with the Manager – Therapy Services to ensure the efficient and effective delivery of program activities, as well as provide leadership in high risk social situations, program development, advocacy and community linkages.

Key responsibilities include providing comprehensive psychosocial assessments, developing and implementing individualized plans of care, providing case management and support, and engaging in continuous quality improvement initiatives.

  • Maintains current working knowledge of relevant legislation and regulations
  • Provides psychosocial support and counseling to individuals and families
  • Develops and implements individualized plans of care
  • Coordinates with healthcare professionals and community agencies to ensure seamless care transitions
