Role Overview:

The Primary Care Network Coordinator will play a key role in supporting the operational success of the PCN team. This position requires a highly organized individual with strong project coordination skills and proficiency in collaborative technologies.

Main Tasks:
  • Develop and implement project work plans, coordinate PCN activities.
  • Track project timelines, gather updates from stakeholders, and report internally.
  • Proactively identify potential project risks, propose solutions, and streamline operational processes.

Eligibility Criteria:
  • Post-secondary degree or diploma program.
  • Minimum of five years of recent, related experience.
  • Valid driver's license and access to a reliable vehicle preferred.

What We Offer:
A full-time, term position (37.5 hours per week) through March 31st, 2027. The successful candidate must have access to a home office or other remote work location.
