We are seeking a highly skilled and experienced Nurse Practitioner to join our team at the Thunder Bay Regional Health Sciences Centre. As a key member of our interprofessional healthcare team, you will provide high-quality care to patients with kidney disease, focusing on hemodialysis management.
Key Responsibilities- Provide advanced clinical assessment, diagnosis, and treatment of patients with kidney disease
- Develop and implement individualized care plans in collaboration with the healthcare team
- Lead and participate in interdisciplinary care planning and coordination
- Collaborate with nephrologists and other healthcare professionals to ensure comprehensive patient care
- Conduct health education and promotion activities to support patients and families
- Participate in quality improvement initiatives to enhance patient outcomes and care processes
- Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario
- Membership in the Nurse Practitioners' Association of Ontario
- Successful completion of graduate studies in nursing
- Minimum 3 years of recent Nurse Practitioner experience in an acute care setting
- Knowledge of and adherence to the Hospital's Code of Conduct
- Demonstrated commitment to the principles of Patient and Family Centred Care
- Clinical knowledge and expertise in chronic kidney disease management
- Previous experience in adult education and program development, implementation, and evaluation
- Strong leadership and interpersonal skills
- Ability to work collaboratively within an interprofessional team
- Excellent communication, negotiation, and conflict resolution skills
This is a temporary full-time position, and the successful candidate will be required to work in a fast-paced, dynamic environment. The ideal candidate will be able to adapt to changing situations and priorities, while maintaining a high level of professionalism and patient care.
