Job Summary

We are seeking a skilled Geriatric Care Manager to join our team at Fraser Health. As a key member of our multidisciplinary team, you will provide comprehensive care management services to frail older adults and adults with complex, acute, and chronic conditions in their homes and communities.

Key Responsibilities
  • Conduct thorough assessments of clients' needs and develop individualized care plans to optimize their capabilities and community engagement.
  • Collaborate with clients, families, healthcare professionals, and community partners to ensure seamless care transitions and minimize re-admissions to residential and acute care facilities.
  • Provide direct client care, coaching, and education to empower clients and their families to manage their health and well-being.
  • Document assessments, care provision, and client responses according to professional standards and guidelines.
  • Participate in quality improvement initiatives and contribute to the development of standards of care, policies, and procedures.
Requirements
  • Bachelor's degree in Social Work from an approved school.
  • Two years' recent, related clinical experience working with complex geriatric clients and/or adults with chronic illnesses in a community health setting.
  • Current full registration with the BC College of Social Workers.
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes.
Preferred Qualifications
  • Demonstrated knowledge, skills, and competence in gerontology, geriatrics, and adults living with chronic illnesses.
  • Ability to communicate effectively, both verbally and in writing.
  • Knowledge of chronic disease management models and community resources.
