About the Role

We are seeking a skilled and compassionate Personal Support Worker to join our team at Transitions Care, Inc. as a Nursing Care Support Worker. In this role, you will provide high-quality care and support to our clients in their homes, ensuring their comfort, safety, and well-being.

Key Responsibilities
  • Assist with daily living activities, such as bathing, dressing, and grooming
  • Provide nursing care, including taking vital signs, administering medications, and performing wound care
  • Assist with mobility and transfers, including walking, lifting, and positioning
  • Prepare and serve meals and snacks, and assist with feeding
  • Maintain a safe and clean environment, including laundry, cleaning, and disinfecting
  • Communicate effectively with clients, families, and healthcare professionals
  • Document client information and progress
Requirements
  • Secondary (high) school graduation certificate
  • Will train
  • First Aid Certificate
  • CPR Certificate
Work Environment

This is a temporary, on-the-road position, requiring flexibility and adaptability. You will work in various locations, including clients' homes and healthcare facilities.
