Job Summary:

The Geriatric Social Worker will work with our community to provide support and services to seniors and their families. This includes assessing client needs, developing and implementing care plans, and providing counseling and therapy to help clients resolve social and personal problems.

Responsibilities:
  • Assess client needs and develop and implement care plans
  • Provide counseling and therapy to help clients resolve social and personal problems
  • Supervise other social workers, paid para-professional staff, student trainees, and volunteers
  • Perform administrative tasks
  • Refer clients to agencies that provide financial assistance, legal aid, housing, medical treatment, and other services
  • Serve as a member of an interdisciplinary team of professionals working with a client group
  • Advocate on client's behalf in courts or at hearings
  • Lobby for solutions to problems affecting client groups
  • Develop prevention and intervention programs to meet community needs
  • Pick up and deliver materials
  • Provide specialized programs for children, seniors, and other groups
