We are seeking a skilled and compassionate Social Service Professional to join our team, responsible for providing essential support services to our residents. As a key member of our staff, you will play a vital role in ensuring our residents receive the highest level of care and attention.

Key Responsibilities

* Develop, implement, and evaluate social service programs tailored to meet the unique needs of our residents
* Provide individualized support and counseling to residents, families, and friends
* Facilitate group sessions and workshops on topics such as grief, coping mechanisms, and stress management
* Collaborate with healthcare professionals to develop and implement personalized care plans
* Communicate effectively with residents, family members, and caregivers to ensure seamless care delivery

Requirements

* Bachelor's degree in Social Work or related field
* Current registration with the Ontario College of Social Workers and Social Service Workers
* Minimum 2 years of experience working in long-term care or a similar setting
* Strong understanding of gerontology, mental health, and social services
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
